Nurse Practitioner – District Stroke Program

Are you a qualified, collaborative Nurse Practitioner with recent clinical neuroscience and/or cardiovascular experience supporting a population of patients with stroke? If so, bring your expertise to this exciting full-time position at Niagara Health’s Greater Niagara General Site.

As a Nurse Practitioner (Stroke Specialist), you’ll provide leadership and coordination of services for patients with acute stroke, as well as primary care to the stroke population in collaboration with the Medical Director of the District Stroke Program. We’ll count on you, as a Stroke Specialist, to facilitate the initiation of Acute Stroke Best Practices in coordination with the District Stroke Centre mandate, and to develop and implement policies and procedures to enhance stroke care in Niagara.

Playing a key collaborative role as Nurse Practitioner (Stroke Specialist), you’ll work closely with the interdisciplinary team to ensure continuity of practice across the stroke continuum within Niagara Health, collaborate with community partners as well as District and Regional Centres to ensure continuity of stroke care beyond our walls, and facilitate stroke care research within Niagara Region.

While your home base will be our Greater Niagara General Site, you’ll be expected to travel between sites.

To join our leadership team as a Nurse Practitioner – District Stroke Program, you must be an experienced and qualified NP whose knowledge base and skill set include:

  • Current registration with the College of Nurses of Ontario - Extended Class.
  • Master of Science in Nursing with a clinical focus.
  • Successful completion of a Nurse Practitioner program (Primary Care or Adult) from a recognized university.
  • Current BCLS.
  • Three (3) years, within the past five (5) years, of recent clinical experience in neuroscience / cardiovascular supporting a population of patients with stroke.
  • Exemplary teaching and leadership abilities.
  • Proven ability to integrate research into practice.
  • A self-directed approach in clinical practice, as required with the stroke population.
  • Demonstrated understanding of the existing processes and mechanisms related to the continuum of stroke care as outlined in the Canadian Stroke best practice recommendations.
  • Proven ability to be supportive of the principles of patient- and family-centred care.
  • Proven ability to be supportive of organizational and departmental goals and objectives, demonstrating initiative to promote total quality improvement.
  • Demonstrated commitment to providing quality customer service while dealing with the public (patients and families), as well as various medical and non-medical professionals.
  • Demonstrated ability to work independently and as a member of a team.
  • Evidence of effective organizational, interpersonal, communication, judgment, critical thinking and decision-making skills.
  • Willingness to work flexible hours (days and evenings) with an on-call commitment, and to travel between sites.
  • Knowledge of data interpretation and strong computer skills.
  • Demonstrated commitment to continuing education.
  • Demonstrated commitment to the mission and values of Niagara Health.
  • Ability to maintain consistent, regular attendance.
