POSITION SUMMARY
This position provides a service, which includes the assessment, planning, implementation and evaluation of home care needs for children/youth and families requiring health care following discharge from CHEO and/or in the community. Discharge planning services are provided in consultation and in collaboration with all members of the health care team and with community agencies. This position carries with it, a commitment to family centered/multicultural care, multidisciplinary teamwork and evidence-based discharge planning practices. This position is for the after hours operations (Monday to Friday 4:00 pm – 8:00 pm; Saturday/Sunday & STATS 8:00AM -8:00PM) During the first couple of months, availability during the day for training is required.
Description of Duties:
- In collaboration with patients and their families/caregivers, assess care needs, determine eligibility for services, and develop individual care and service plans.
- Promotes early identification and assessment of the patient’s needs.
- Coordinate and monitor care plan delivery
- Implements timely discharge plans that enhance the continuity of care and efficient use hospital and community resources.
- Establishes and maintains productive communication between hospital and community resources.
- Initiates follow-up to evaluate effectiveness of care plan.
- Utilizes nursing research to enhance the quality of patient care both as an in-patient and following discharge.
- Acts as an educator and resource person to all hospital departments and a wide variety of community agencies.
- Completes documentation and maintains appropriate EPIC and CHRIS patient records in accordance with the employee’s respective regulatory College and CHEO policies and procedures.
- Attends and participates actively in team meetings and general staff meetings.
- Assumes responsibility for maintaining professional knowledge and updated skills.
- Will support other duties as assigned.
QUALIFICATIONS
- Criminal Records Check (Essential)
- Degree in Nursing (RN), Physiotherapy, Occupational Therapy, Speech-Language Pathology, Social Work or Dietetics (Essential)
- Current registration with the appropriate regulatory College (Essential)
- At least 2 years of related professional experience (Essential)
- Related experience in a community-based setting (Preferred)
- Demonstrated competency in assessment of pediatric health care needs (Essential)
- Knowledge of medical diagnoses and prognoses, symptom management, and cognitive/psychiatric disorders (Essential)
- Awareness of and sensitivity to the needs of a culturally diverse population (Essential)
- Communicates effectively, orally and in writing (Essential)
- Demonstrates excellent in interpersonal skills (Essential)
- Excellent time management and organizational skills (Essential)
- Works effectively in both a team environment and independently as required (Essential)
- Diplomacy in dealing with sensitive and/or confidential issues (Essential)
- Bilingualism (English and French) (Essential)
