Position Title: Full-Scope Community Health Nurse (CHN)

Location: Easterville/Chemawawin

 

Roles

The Community Health Nurse will act as a essential component of healthcare in Easterville/Chemawawin, taking on leadership and care delivery roles. The ideal candidate is resilient, resourceful, and committed to holistic, patient- and community-centred care.

  1. Healthcare Provider
    • Deliver full-scope maternal and child health services, including prenatal, newborn, and postpartum care.
    • Conduct immunizations, and developmental assessments, and provide ongoing health monitoring for children and adolescents.
  2. Youth Advocate & Violence Prevention Specialist
    • Build long-term trust with children aged 8-12, creating a foundation for effective care.
    • Lead violence prevention initiatives in schools and the community, emphasizing early intervention.
    • Collaborate with schools, families, and community organizations to support at-risk youth.
  3. Community Health Leader
    • Champion healthcare integration with community services like education, housing, and social support to address the root causes of health issues.
    • Act as a mentor to other healthcare providers and workers, expanding community capacity to address health concerns.
    • Design and implement health promotion activities targeting mental health, substance use, and chronic disease prevention.
  4. Program Innovator
    • Develop, assess, and refine health initiatives tailored to the community’s needs.
    • Monitor emerging health trends, conduct regular assessments, and provide data-driven recommendations to improve community health outcomes.

 

Responsibilities

The Community Health Nurse will handle key tasks that impact the health and well-being of the community, focusing on patient-centred care and preventive initiatives.

  1. Maternal and Child Health
    • Provide comprehensive prenatal care, newborn assessments, and postpartum support.
    • Ensure all children receive timely immunizations and developmental screenings.
    • Refer families to additional resources or specialists as needed.
  2. Youth Engagement & Violence Prevention
    • Develop trusting relationships with children early on to ensure continuity of care.
    • Facilitate school-based education on violence prevention, self-esteem, and sexual health.
    • Identify and support at-risk youth in partnership with mental health professionals and social services.
  3. Community Collaboration
    • Organize and lead community-wide health promotion events and workshops.
    • Address social determinants of health by collaborating with housing, education, and recreation sectors.
    • Act as a liaison between the community and external healthcare services.
  4. Leadership & Advocacy
    • Work closely with the Nurse Practitioner, doctors, and other team members to ensure seamless care.
    • Guide community health workers and mentor other staff to improve overall capacity and effectiveness.

The Community Health Nurse is expected to achieve measurable improvements in community health outcomes:

  1. Enhanced Maternal and Child Health
    • Increased prenatal visit rates and improved maternal and infant health outcomes.
    • All children in the community are vaccinated on schedule and meet developmental milestones.
  2. Youth Violence and Risk Behaviour Reduction
    • Decreased rates of youth violence and improved school engagement.
    • Increased sexual health knowledge among adolescents, reducing rates of teen pregnancies and STBBIs.
  3. Improved Community Health Literacy
    • Increased participation in health promotion activities.
    • Strengthened collaboration among healthcare providers and social services.
  4. Sustainable Health Initiatives
    • Long-term health programs are tailored to the unique needs of the community.
    • Data-driven evaluation to ensure cost-effectiveness and continued support for programs.

 

Requirements

  1. Education & Certification
    • Bachelor’s degree in Nursing (BSN) from an accredited institution.
    • Public or Community Health Nursing Certification is an asset.
    • Active registration with the College of Registered Nurses of Manitoba (CRNM).
  2. Experience
    • 5-10 years in community or public health nursing, focusing on maternal and child health.
    • Experience in northern or remote communities is highly desirable.
    • Proven leadership in health promotion and community engagement.
  3. Skills
    • Strong communication and interpersonal skills to build trust with diverse populations.
    • Ability to lead programs and make data-driven decisions.
    • Cultural competence and sensitivity to the unique needs of the northern community.
