Job Summary:

Within the context of a client and family centered model and in accordance with the Saskatchewan Registered Nurses Association (SRNA) standards of professional practice, Code of Ethics and the established vision and values of Saskatchewan Health Authority (SHA), the Palliative Care Coordinator provides assessment and access to a comprehensive range of high quality End-Of-Life-Care for patients admitted to palliative care services in any care setting and their family/supporters.

Under the direction of the program area manager, the Palliative Care Coordinator designs, implements and evaluates palliative care services provided to clients in the Saskatchewan Health Authority.

 

Qualifications:

Required Qualifications

  • Baccalaureate Degree in Nursing
  • Canadian Hospice Palliative Care Nurse Certification (CHPCN) recommended or willing to pursue when eligible
  • Current registration in good standing with College of Registered Nurses of Saskatchewan (CRNS)
  • LEAP certification (Learning Essential Approaches to Palliative and End of Life)
  • Must possess a valid Saskatchewan driver’s license and have access to a reliable vehicle
  • Three years related experience in Palliative Care (combination of Acute and Community preferable)

 

Knowledge, Skills & Abilities

  • Ability to demonstrate strong clinical assessment, decision making, and critical thinking
  • Ability to establish and maintain positive relationships with all stakeholders who are supporting palliative clients and families
  • Ability to operate computers and related equipment including software applications
  • Ability to provide clinical recommendations to health care providers, clients and their caregiver
  • Demonstrated ability to plan, organize and prioritize work within a continuously changing environment
  • Demonstrated ability to provide leadership by supporting best practice to deliver high quality palliative care services
  • Demonstrated ability to resolve problems and effectively manage conflict situations
  • Demonstrated commitment to continuing professional development
  • Proven ability to articulate and integrate theory, knowledge and experience into activities to enhance client centered care
  • Proven ability to display a high degree of emotional maturity to interact with numerous patients and families in varying degrees of denial or acceptance of the medical outcome for themselves or their loved one
  • Proven ability to provide leadership to physicians, staff, volunteers and community participants using modeling, coaching, mentoring and adult learning principles
  • Proven ability to work independently, set priorities and show initiative
  • Proven excellent communication skills, both verbal and written

 

Additional Text:

This is a designated field hours position. This is a regional position currently based in Meadow Lake. Coverage would be expected for Goodsoil, Loon Lake, Pierceland, and the former Keewatin Yatthe region.


Successful candidate may be eligible for up to $3,000.00 in relocation assistance.
