We are seeking a conscientious, experienced, forward thinking clinical leader to join our journey as a full-time Clinical Manager.

 

Job Summary

The Clinical Manager, reporting to the Director of Clinical Services, is responsible for the delivery of care and services provided to clients. The Clinical Manager is responsible for providing overall clinical operational leadership, planning effective and individualized high quality service to clients in the community setting. Amalgamating with various stakeholders, both internally and externally, the Clinical Manager will ensure that families and clients receive the highest quality service and delivery of care. The Clinical Manager will serve as the clinical expert and support care coordination, human resources, scheduling, and care advisors. The Clinical Manager develops individualized plans of care, provides clinical guidance to care providers and coordinates with allied health professionals. The ideal candidate will have strong clinical experience and have a strong foundation of an informatics approach to transform healthcare delivery.

 

Duties and Responsibilities

  • Assess the initial and ongoing assessment of clients requiring care.

  • Develop individualized plans of care for clients and ensure proper utilization of the biopsychosocial model.

  • Visit clients upon admission and as necessary to adjust plans of care.

  • Support and supervise field staff to ensure compliance with Mavencare policies and procedures and federal/provincial standards.

  • Provide in-services and education to field staff.

  • Coordinate and assist human resources with recruitment, orientation, and training of field employees.

  • Liaise with other healthcare providers, and community agencies as required.

  • Promote Mavencare through participation in local healthcare events.

 

Qualifications

  • Registered Nurse license in the province in Ontario, and in good standing

  • Graduated from an accredited university with a BSN

  • Minimum 2 years experience in home care and geriatric care

  • Previous supervisory or leadership experience an asset

  • Ability to work effectively to address challenges as part of an interdisciplinary team

  • Understanding of the scope of practice of personal support workers.

  • Comfortable presenting to groups of clients or employees of the agency.

  • Strong verbal and written communication skills.

  • Excellent time management and multitasking skills.

  • Valid G License and reliable means of transportation.
