Position: Clinic Nurse
Position Summary
We are seeking a highly motivated nurse to provide direct clinical care & care coordination in a primary care setting. The ideal nurse for this role needs to exhibit strong critical thinking, decision making, clinical assessment abilities, interpersonal skills, written communication, as well as the ability to work independently and as part of a team.
In this role you will help patients, and their families, understand diagnostic test results, consult reports and next steps regarding their care plan. You will be responsible for processing health records, so they are available and exchanged appropriately with all relevant members of the care team, particularly with patients and their families. You will empower patients and their families to become active members in their healthcare by providing education, support, resources and assisting them to navigate through the healthcare system.

In this role you will work in-clinic Monday to Friday 0900-1700 at our Midtown Toronto clinic. Holidays and weekends off.
Qualifications and Skills
- Registered Nurse or Registered Practical Nurse registered in good standing with the College of Nurses of Ontario.
- Current BLS Certificate required, patient assessment care course will be seen as an asset.
- Preferred acute care nursing experience (i.e.. Emergency, community, mental health, ICU)
- Demonstrated Nursing experience with the elderly population and persons with complex needs (i.e. mental health illnesses, addictions, diabetes, cardiovascular conditions)
- Ability to engage with families and comfortable navigating challenging conversations with compassion and empathy
- Ability to work effectively independently and collaboratively within an interprofessional team while multitasking and managing multiple priorities
- Excellent patient assessment and teaching skills
- Ability to perform ECG’s & phlebotomy or willingness to learn
- Proficiency in English, both verbal and written, is required. Fluency in written and oral French or other language is considered an asset
- Competency using technology, telephones, software and computer programs
- Satisfactory attendance record and up-to-date Police Reference Check
