Program Overview

Consistent with principles of primary health care Behavioural Health Consultants assess, treat and manage patients referred for mental health and chronic disease concerns. The BHC will work with patients of all ages from a biopsychosocial perspective at their current stage of change, all strategies will be based on brief interventions to improve self-management and be focused on symptom reduction and improved functioning. Behavioural Health Consultants also help the physician and patient navigate PCN or other formal and informal external connection options when necessary.

Position

The Behavioural Health Consultant utilizes the Behavioural Health Consultant model of care to complete standardized functional assessments to identify functional concerns, current risk and readiness for behavioural change. Operates as a resource to other multidisciplinary team members, healthcare institutions and community agencies or programs to support the delivery of high-quality, evidence-based health care to the practice population.

Team Lead, Mental Health Services

Education

Bachelor’s Degree in Psychiatric Nursing, Nursing, Social Work or Occupational Must have an active professional registration with a relevant college as recognized by the Health Professions Act *may require psychosocial intervention authorization

Experience

Previous experience in primary care and/or community-based setting, preferably with a mental health focus

Experience with class facilitation/instruction, crisis management and   a strong awareness of community-based resources

*candidates that have a working knowledge of motivational interviewing, solution-focused therapy, CBT, and strong suicide risk assessment skills are preferred

Skills

Strong communication skills, adaptability, and integrity.

Must be able to organize time, multi-task and set priorities to meet various deadlines and maintain a flexible schedule

Values collaboration and contributes to the development and implementation of community-based solutions to meet the mental health needs of the population.

Days/Hours of Work

38.75 hrs/week with the requirement of one (1) evening shift per week (11:45a.m.- 08:00p.m.).
