Home Care Nurse

Metlakatla First Nation has an immediate opening for a full-time Home Care Nurse to provide services to community residents in Metlakatla, British Columbia.  As the successful candidate, you will be responsible for nursing services that support an integrated, comprehensive case-managed home care program for identified individuals and families. The incumbent will work collegially with other professionals and paraprofessionals to assess, plan, implement, deliver and evaluate services for clients.

 

Responsibilities:

  • Provide nursing services to the Home and Community Care Program and work collegially with the paraprofessional staff;
  • Provide professional services on a visiting basis to assist home care clients of all ages and their families in achieving and maximizing their independence;
  • Acting as the primary intake worker for home care referrals;
  • Provide support for clients and families in managing home situations;
  • Completing initial assessment and coordinating other required assessments, i.e., nursing, etc.;
  • Provide functional support and guidance to the Community Health Representative (CHR) and other staff members to ensure services are delivered appropriately and effectively;
  • Participate in the development and implementation of the Home and Community Care Program to assist in the delivery of quality home care services;
  • Maintain professional competency and act as a resource person to professionals, paraprofessionals and the community. As a champion of the Home and Community Care Program, promote self-care, independence and awareness of the program;
  • Deliver targeted, culturally appropriate information to help individuals and groups understand local environmental public health policies, regulations and code;
  • Conduct Quarterly activity review (programs & projects) and prepare monthly reports outlining activities, statistics on clients served, outline for following month activities;
  • The position assists in Strategic Planning, budget development for the home care aspect of the Community Care Program, Annual Department Work Plans and Staff Individual Training Plans;
  • Perform other duties within the scope of the position, as required.

 

Required Knowledge, Skills and Abilities:

  • Current practicing registration with the College of Registered Nurses of BC (CRNBC);
  • Two (2) years' recent clinical experience that is relevant to the specific clinical area of employment;
  • Diabetes Educator Certification is a definite asset;
  • Specific skill and knowledge of community health nursing theory and practice within a client and family-centred model of care;
  • Broad knowledge of the CRNBC standards of practice and guidelines for clinical practitioners;
  • Knowledge of the Community Health Nurse of Canada (CHNC) competencies and standards of practice;
  • Ability to promote client-focused care that demonstrates care for clients and significant others, sensitivity to diverse cultures and preferences, client advocacy and social justice concerns;
  • Basic knowledge of nursing standardized tests and measures;
  • Knowledge and skill in the provision of evidence-based clinical practice related to designated client populations;
  • Knowledge of provincial acts, regulations, and program policies and guidelines related to home and community care;
  • Knowledge of acute, chronic disease, palliative and health management and self-management support;
  • Knowledge of community resources available for client/family support and related health services.
