Registered Nurse

Position Profile:

This position provides direct primary care within their full scope of practice and participates in the development, implementation, monitoring and evaluation of services for individuals and families. The RN works collaboratively as a member of an interdisciplinary team and must include the social determinants of health in their assessments and interventions with individuals, families, and communities. This is practiced with a variety of professionals in diverse care settings such as institutions or a home-care environment.

Key Responsibilities:

  • Arrange and/or provide client care by utilizing the skills of observation, assessment, nursing diagnosis, counseling and health teaching; report any unusual circumstances to the branch office or to the immediate supervisor.
  • Provides nursing care to residents, develops, evaluates, and modifies care plans, participates in care planning, and works collaboratively with team members to deliver person-centered care with an emphasis on resident/client safety and engagement. Procure and administer medications including all injections and narcotics.
  • Observes, monitors, and evaluates residents’ conditions; reports vital signs, injuries, and general mental and physical condition, attending immediately to critical incidents involving residents.
  • Documents and reports in resident chart condition report in a timely manner to ensure complete and accurate resident records.
  • Overviewing the information updated by the Personal Support Workers in management of the residents’ care requirements in logs, POC, other documents.
  • Provide direction, support, and delegation to Licensed/Registered Practical Nurses and unlicensed Field employees as appropriate.
  • Consider the age of the patient and his or her emotional stability when determining appropriate treatment.
  • Track and maintain medical supply inventory along with other staff members.
  • Counsel patients and family members before, during and after treatment.
  • Other duties as assigned by the supervisor or devised under the specific care plan (Getting approved if needed).

Skills & Qualifications:

  • Current Annual Registration as an RN with the College of Nurses of Ontario.
  • Recent Criminal record check (Vulnerable Sector) and 2 Step T.B. Test/Chest x-ray results required, along with current CPR Level C and First Aid documents and valid eligibility to work in Canada.
  • Understanding of the requirements under the Long-Term Care Act, CNO Standards, and other regulations relevant to Long Term Care and Community Health.
  • Good verbal and written communication skills, using tact, diplomacy, and empathy to share difficult information with a sensitive and compassionate approach, high level of detail orientation.
  • Ability to work independently and in a team based, collaborative fashion handling multiple tasks safely and efficiently.
  • Basic computer skills.
  • Demonstrated ability to handle critical situations in a practical, logical fashion.
  • Gentle Persuasive Approach, Non-Violent Crisis Intervention, and other relevant training an asset.
  • Previous experience working with the senior demographic a definite asset.
  • Bilingual preferred.
  • If required, current drivers’ license, proof of insurability and clean driving abstract, access to a reliable vehicle.
  • Ability to meet the physical requirements of the job – e.g., long periods of standing/walking, pushing/pulling, and physical lifting capabilities, and ability to help clients to reposition and transfers.
