Registered Nurse - Diabetes Educator

This full-time position, which reports to the Medical Director and the Chief Operating Officer, will administer the diabetes education program for outpatients at our clinic. The Certified Diabetes Educator is accountable to obtain appropriate physician consultation according to the Expectations for Consultation with Physicians by Registered Nurses.

  • Provide education related to diabetes and the disease process, medication, diet and nutritional monitoring, physical activity, foot care, prevention and care of complications, blood glucose monitoring and the techniques of self-monitoring and injection for both insulin and non-insulin dependent diabetics
  • Document assessments, interventions and plans including recommendations, goals and progress notes
  • Utilize a team approach to provide services and collaborate and communicate with team members when needed
  • Provide diabetes education program (DEP) to groups and
    • Collect DEP patient assessment data, in a collaborative and ongoing manner
    • Document assessment data, educational plan, educational services provided and evaluation results in each patient’s records
    • Identify when a DEP patient’s needs are outside the scope of the facilitator’s practice and expertise, plus arrange for additional services to meet needs
  • Facilitate the coordination of foot care groups and clinics
  • Provide diabetes resource and consultation to physicians
  • Identify and initiate plans to address service gaps between existing diabetic health service practices and optimal standards of care
  • Provide on-going reviews and updates of the diabetes health program policies and procedures; develop and implement quality improvement processes
  • Promote the mission, vision, and values of the organization
  • Process compassion requests, special authorization with PharmaCare and external healthcare providers, and liaise with drug companies
  • Coordinate educational talks for physicians and staff

Requirements :

 

Technical

  • Registered Nurse in good standing with the College of Nurses of B.C.
  • Minimum of 3-years of experience in a RN capacity
  • Certified Diabetes Educator (or willing to complete in next 4 months)
  • Experience working in a medically assisted weight loss clinic considered an asset
  • Excellent communication and presentation skills
  • Proficient with computer applications including Word, Excel and PowerPoint
