Registered Nurse/Licensed Practical Nurse

Summary of the Role Responsibilities 

Rotary Hospice House (RHH)/ Home Hospice Nurses assess, identify, plan, implement and evaluate the nursing care required to assist clients living at home; in meeting their physical, social, spiritual and psychological needs. RNs/LPNs work within an interdisciplinary clinical framework in accordance with the BCCNP Standards of Practice and consistent with the mission, vision and values of The Prince George Hospice Palliative Care Society.  

Functional Reporting Relationships 

• Reports to Nurse Lead and RHH Manager.  

Duties and Responsibilities 

• Liaise with Health Care physician 

• Evaluate client status by reviewing documentation, care plan, client observation, collateral information and consultation with other healthcare team members. 

• Assesses, plans implements, and evaluates nursing care for clients using the framework for nursing practice. Administers medications manages basic to complex wounds, catheter insertion (and manages feeding tubes, ostomies and other medically provided devices). Documents in electronic charting program. 

• Creates individual care plans through methods such as assessing client problems and needs, planning and prioritizing nursing interventions, implementing interventions and reassessing if goals are not met and modifying the plan of care based on the evaluation. 

• Work collaboratively with patients, family and care providers in developing Care Plan. 

• Provides nursing care according to identified policies and procedures, technology and PHC standards; evaluates outcomes, records observations and reports problems and changes to designated staff. 

• Provides care according to the BC Centre for Palliative Care symptom guidelines 

• Provides care in accordance with the Hospice philosophy of care.  

• Identifies client and family learning needs, incorporates teaching and counselling into the care plan and nursing interventions, and makes referrals to other identified resources. 

• Responsibilities include but are not limited to collegial sharing of information, demonstrating and providing guidance on related tasks. 

• Provides direct nursing interventions for the management of crisis and life-threatening situations. 

• Documents information including assessments, nursing care plans, medication administration, client status and progress and discharge plans in a computerized environment. 

• Participates in nursing orientation and shares clinical knowledge 

• Actively participates in direct patient care. 

CORE COMPETENCIES 

Required Licences, Certificates, and Education 

• Registered Nurse with current practicing registration with the BC College of Nursing Professionals BCCNP 

• Pallium Canada LEAP Training is required (may be provided)  

• Minimum one (1) year of recent, related palliative nursing care experience in an acute care facility or an equivalent combination of education, training and experience.  

• Level 1 First Aid 

• Regular Class 5 Driver's License. 

• Ongoing orientation/training as required.
