Coordinator - Geriatric Services

Job Summary:

Reporting to the Director of Geriatric Services, this position provides supervision and direction for the day‑to‑day activities of the inter‑professional Geriatric Services Resource Team (GSRT) and coordinates their activities in the delivery of services to the geriatric population across southern Saskatchewan in the Saskatchewan Health Authority.  In addition to triaging referrals to Geriatric Services, the incumbent will be responsible for identifying high risk situations that require priority attention, initiating call‑backs to referrals to obtain information that is required for triaging work, assisting with workloads by completing some assessments that have been initiated by referrals for responsive behaviors, liaising with physicians and other professionals, and supporting the outpatient clinics by collecting/collating information in preparations for outpatient consultations, assessing clients  and coordinating follow up for clients after the outpatient clinic.

The main responsibilities are to:  facilitate effective organizational planning for the GSRT members' daily work, supporting the team with evidence based knowledge that is best practice mitigating potential risks by prioritizing the order in which referrals are responded to, coordinating outpatient clinic referrals and follow up, helping to balance the work load that is generated by the referrals and supporting processes within the organization which lead to continuous quality improvement.

Qualifications:

Education:

Graduate from an approved Nursing Program.  Baccalaureate Degree in Nursing.  P.I.E.C.E.S. training is an asset.  Possession of, and ability to maintain, a current registration with the Saskatchewan Registered Nurses Association (SRNA) or Registered Psychiatric Nurses Association (RPNAS) of Saskatchewan prior to employment.

Experience:

A minimum of five (5) years recent experience related to clinical areas in geriatrics and dementia.  A minimum of three (3) years, in the past five years, in a supervisory/leadership position would be an asset.  Experience in program/resource development related to needs assessment, program decision implementation and evaluation, facilitation skills including leading group activities.

Knowledge, Skills and Abilities:

Demonstrated knowledge of geriatric physiology and how medical conditions impact function.  Demonstrated knowledge regarding the delivery of long term care.  Demonstrated knowledge of community resources, programs and services in Saskatchewan.

 Demonstrated knowledge of dementia and underlying medical causes for reversible dementia.  Demonstrated knowledge of best practices for geriatric clients.  Proven ability to mentor, support knowledge transfer, and assist staff in meeting complex care needs of the geriatric population, including clients with dementia.  Demonstrated organizational problem solving and decision making skills.  Excellent interpersonal skills.  Proven skill in basic computer operations, word processing and spreadsheets.

 Demonstrated ability to work independently as well as collaboratively with colleagues, other regional portfolios, external agencies, provincial bodies.  Demonstrated ability to provide effective leadership by planning, supervising and directing work.

 Demonstrated ability to facilitate good staff morale.  Demonstrated ability to communicate effectively both oral and written.

 

Additional Information:

This position is field hours.  Must possess a valid driver's license.  CVA vehicles are utilized for travel related to regional business.  Travel allowance is provided if utilization of personal vehicle is occasionally required.  Please submit a cover letter and resume when applying for this position.  Scheduled hours of work may be reduced (if applicable) during the rotation stipulated when a statutory holiday is recognized during that period.
