Reporting to the Director of Care, the Resident Care Coordinator is responsible for the clinical supervision of staff and programs impacting health care delivery, including, but not limited to: BC Regional Health Authorities' Community Care Access Review Processes and Admission Programmes, Assessments, Care Planning, Orientation, In-Service Education, Infection Control, Quality Assurance Reviewing and Auditing, and the MDS, POC, PCC, EMAR and PIECES Programmes that are the basis of Person-Centred Care.

Responsibilities


. Ensures resident satisfaction and continuous quality improvement initiatives are promoted;
. Works alongside nursing staff to ensure that initial/ongoing assessments and care plans are developed, implemented and reviewed;
. Hires, orientates, directs, coaches, supervises, evaluates, promotes, disciplines and terminates staff as required;
. Ensures quality care and a safe, effective and efficient work environment;
. Completes MDS and associated assessments as required, and maintains AIS coding competency annually;
. Works with POC, PCC, eMAR and PIECES programmes;
. Attends interdisciplinary care conferences as required;
. Provides ongoing staff development and education to improve or enhance the level of care and service provide to the Residents, which includes planning, coordinating, conducting and evaluating in-service education sessions;
. Responsible for the health and safety of the home, residents, staff and guests.
o Is aware of employees' responsibilities and follows all health and safety policies and procedures as set out in the employee safety program handbook and departmental manuals.
o Provides first aid to staff as required.
o Works safely to reduce the risk of injury to self, co-workers and residents.
o Is alert to and promptly reports all actual or potentially hazardous situations to immediate supervisor.  Does not operate or use faulty equipment.  Deals with safety issues as needed.
o Wears personal protective equipment (or clothing) as required by task, M.S.D.S. or home policy.
o Promptly reports personal injury to immediate supervisor and seeks first aid as needed.
o Participates in fire safety demonstrations and fire drills, and knows the fire and disaster plan.
. Works with the Director of Care in the implementation and monitoring of the Nursing Department Quality Assurance Program and Quality and Compliance Review and Auditing Program;
. Participates in committees and prepares reports as needed;
. Adheres to established policies and procedures regarding quality assurance, fire, safety, environment and infection control and participates in ensuring compliance by staff members;
. Performs other related duties as required.

Key Relationships
Reports to: Director of Care
Manages: the Nursing Department in collaboration with the Director of Care
Interfaces and builds relationships with: staff, Residents, families, health authorities, Licensing, Schools of Nursing and Care Aides, allied professionals

Customer Satisfaction: Resident/family satisfaction, clinical outcome measures
Financial Performance: budget adherence, MDS-RAI

Working Conditions
May be required to work evenings and weekends as required.

Qualifications


. Licensed Practical Nurse (LPN/RPN) with a valid, practicing nurse licence with the applicable provincial licensing body;
. Several year experience as a Nurse in the long-term care sector;
. Effective oral and written communication, interpersonal and problem-solving skills;
. Strong motivational, team building and time management skills;
. Knowledge and experience with MDS/RAI, PCC, POC, and eMAR required;
. Experience with CQI preferred;
. Experience with and knowledge of staff education;
. Current possesses and maintains certification in Occupational First Aid Level 1, and Care Provider CPR;
. Maintains wound care management annually;
. Knowledge of Island Health (VIHA) and/or Island Health's Community Care Access Admission processes and Referral Programmes an asset;
. Current knowledge of clinical issues for geriatric residents, infection control and principles of adult education desirable;
. Knowledgeable in principles and philosophy of Long Term Care;
. Knowledgeable of the Resident's Bill of Rights;
. Experience in a unionized environment preferred;
. Knowledge of occupational health and safety practices, procedures and legislation;
. Computer literacy in Microsoft Office;
. Current (within 6 months) Vulnerable Position/Sector Screening (VPS).
