Position Summary

Reporting to the Director of Care, the MDS/RAI Resource Nurse is responsible for coordinating the development and completion of the Resident Assessments (RAI), which include the Minimum Data Set (MDS) care plan, MDS scheduling, additional assessments, act as site wound care champion, and provide direct resident care as required in accordance with best practice guidelines, current Provincial regulations, and site standards and guidelines.

. Coordinating the development and completion of the Resident Assessments (RAI), which include the Minimum Data Set (MDS) care plan, MDS scheduling, in accordance with best practice guidelines, current Provincial regulations, and site standards and guidelines;
. Ensure completion of MDS per RAI guidelines, including dating and signing each MDS to indicate
that the assessment has been completed;
. Ensure that MDS information is accurate and transmitted per guidelines;
. Participate in Interdisciplinary Team, Licensed Nursing Meetings any other meetings deemed
necessary by the DOC;
. Act as a wound champion for the site including attendance, chairing of any requisite committees or meetings;
. Collaborate with the nursing staff to ensure that wounds are appropriately treated and care plan developed in order to reduce wound escalation;
. Ensure wound, fall, and skin integrity reports are completed and submitted on schedule;
. Oversee that the care plan has been established in accordance with Facility and regulatory
guidelines and is reviewed based on the residents MDS and reflects the resident's current condition;
. Coordinate the MDS schedule for needed assessments, with Interdisciplinary Team, to ensure
compliance with all scheduled MDS assessments;
. Participate in stand up meetings on a daily basis, completing resident follow-ups as requested;
. Assure quality nursing care is being provided, complete follow-up with Resident Care Coordinators, Team Leaders, Charge Nurse, Management and nursing staff to ensure the best possible resident care;
. Participate in planning and the admission process to ensure resident's smooth transition in and out of care;
. In collaboration with nursing, order and maintain nursing supplies;
. Participate in quality assurance and performance improvements process to include assigned
monitoring, audits, and unusual occurrences investigations as requested;
. Any other duties as assigned.


. Licensed Practical Nurse (LPN/RPN) with a valid, practicing nurse licence with the applicable provincial licensing body;
. Several year experience as a Nurse in the long-term care sector;
. Effective oral and written communication, interpersonal and problem-solving skills;
. Strong motivational, team building and time management skills;
. Knowledge and experience with MDS/RAI, PCC, POC, and eMAR required;
. Experience with CQI preferred;
. Experience with and knowledge of staff education;
. Current possesses and maintains certification in Occupational First Aid Level 1, and Care Provider CPR;
. Maintains wound care management annually;
. Knowledge of Island Health (VIHA) and/or Island Health's Community Care Access Admission processes and Referral Programmes an asset;
. Current knowledge of clinical issues for geriatric residents, infection control and principles of adult education desirable;
. Knowledgeable in principles and philosophy of Long Term Care;
. Knowledgeable of the Resident's Bill of Rights;
. Experience in a unionized environment preferred;
. Knowledge of occupational health and safety practices, procedures and legislation;
. Computer literacy in Microsoft Office;
. Current (within 6 months) Vulnerable Position/Sector Screening (VPS).

Key Relationships
Reports to: Director of Care
Manages: the Nursing Department in collaboration with the RCC(s) and Director of Care
Interfaces and builds relationships with: staff, Residents, families, health authorities, Licensing, Schools of Nursing and Care Aides, allied professionals

Working Conditions
May be required to work evenings and weekends as required.
